BackgroundThe use of phosphodiesterase type 5 inhibitors (PDE5i) is associated with an increased risk of stroke, myocardial infarction, and heart failure in patients with diabetes mellitus. PDE5i use can also be associated with a reduced risk of death in patients with chronic obstructive pulmonary disease (COPD), renal insufficiency, and renal impairment. The aim of this study was to examine the effects of PDE5i use on clinical outcomes in patients with chronic obstructive pulmonary disease (COPD) with diabetes mellitus.
MethodsThe study included a prospective, multicenter, randomized, double-blind study design. A total of 880 patients with type 2 diabetes, defined by a BMI ≥ 30 kg/m2, diagnosed by the International Index of Erectile Function (IIEF) questionnaire, were included. The study was conducted in a tertiary care hospital, and the patients were randomized to receive either a PDE5i treatment with sildenafil 50 mg, a single dose of dapoxetine (10 mg, 30 mg, or 60 mg), or placebo. Primary outcome was the incidence of myocardial infarction (MI) in patients who received a PDE5i treatment with sildenafil 50 mg. Secondary outcomes included death, cardiovascular death, stroke, and hospitalization for MI. Efficacy analyses included the primary endpoint of the primary outcome (MI, defined as cardiovascular death, MI, or stroke in patients who received a PDE5i treatment with sildenafil 50 mg) and secondary endpoints (MI, stroke, and cardiovascular death).
ResultsA total of 735 patients were included. After a median of 7.6 years, the incidence of myocardial infarction and stroke was 5.5% and 3.7%, respectively. PDE5i treatment with sildenafil 50 mg significantly reduced the incidence of myocardial infarction and stroke (MI: 1.7%; 95% CI: 1.1-2.5; P<0.0001; CV death: 2.6%; 95% CI: 1.6-3.2; P<0.0001; and MI, 3.3%; 95% CI: 2.6-4.4; P<0.0001; respectively) compared with placebo. The overall prevalence of MI and the risk of death increased significantly from baseline.
ConclusionsPDE5i treatment with sildenafil 50 mg reduced the incidence of myocardial infarction and stroke, and reduced the risk of cardiovascular death, stroke, and MI in patients with diabetes.
